Part 1 — Website privacy policy

Mona Gobrail LCPC LLC ("the practice," "we") operates monagobrail.com. This page explains what information the website collects and how it is used.

What the website collects. The site is hosted on Squarespace. Squarespace may collect standard technical information when you visit (such as your IP address, browser type, and pages viewed) to operate the site and for basic analytics. We do not sell this information and do not run advertising on this site.

Contact and intake forms. When you contact us or start an intake, the information you provide (your name, contact details, insurance information, and the answers on the intake questionnaire) is used only to respond to you, to determine whether we are a good fit, and, if you become a client, to set up your care and bill your insurance. Intake forms and signatures are handled by eSignatures.com, which provides secure electronic signature services to the practice under a written agreement. Completed intake forms become part of your clinical record and are protected as described in Part 2.

Connected services the practice uses. To run the practice, we use a small number of software services that handle client information under written privacy and security agreements: Zoom for telehealth sessions (including session recording and transcription when you have agreed to it), Microsoft 365 for secure storage of clinical records, an external medical billing company for insurance claims, and eSignatures.com for intake forms. We use these services only to provide and administer care.

Email and scheduling. If you email us or we schedule with you by email, that correspondence is stored in the practice's email account. Email is not a secure way to send sensitive health information; please keep clinical matters for your sessions.

Children. This website is not directed to children under 13 and we do not knowingly collect information from them online. Services to minors are arranged with a parent or guardian.

Your choices. You may ask us what information we hold about you from your website contact, ask us to correct it, or ask us to delete website-form information that is not part of a clinical record. Clinical records are governed by Part 2 and by Maryland record-retention law.

Changes. We will post any changes to this page here with a new effective date.

Part 2 — Notice of Privacy Practices (summary)

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who we are. Mona Gobrail LCPC LLC is a licensed clinical professional counseling practice in Maryland. We are required by law to keep your health information private, to give you this notice, and to follow it.

How we use and share your information.

  • Treatment. To provide your counseling, including coordinating with another provider involved in your care when you agree.
  • Payment. To bill your insurance company, through our billing company, which requires diagnosis and service information to process claims.
  • Operations. To run the practice, including quality review, supervision or professional consultation (without identifying you where possible), and secure record-keeping.
  • With your written authorization for anything else, which you may revoke in writing at any time.
  • Without your authorization only where the law requires or allows it, such as: when there is a serious and imminent threat to you or others; suspected abuse or neglect of a child or vulnerable adult; a court order; oversight by licensing authorities; or as otherwise required by law.

Telehealth, recordings and documentation. Sessions are held by secure video. With your consent, sessions are recorded and automatically transcribed, and the transcript is used with software tools to prepare a draft clinical note that your therapist reviews and finalizes. Recordings and transcripts are stored securely and are part of your clinical record. You may decline recording, or ask that a recording be stopped, at any time without affecting your care.

Psychotherapy notes. Any separate psychotherapy notes your therapist keeps receive additional protection and are not released for payment or operations without your specific authorization.

Your rights. You have the right to: see and get a copy of your record (with limited exceptions); ask us to correct it; get a list of certain disclosures we have made; ask for restrictions on how we use or share your information; ask us to contact you in a particular way or place; get a paper copy of this notice; and file a complaint. To exercise these rights, contact us at the address above.

Records retention. Maryland law requires us to keep adult records for at least seven years after the last service, and records of minors until seven years after they reach adulthood.

Complaints. If you believe your privacy rights have been violated, you may complain to us or to the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Effective date and changes. This notice is effective as of the date above. We may change it, and the current version will always be available on this page and on request.
